Comments and free-text usually carry the reasoning, and those survive poor discipline in the structured fields.

We have used three different tools.

Common. Fragmentation lowers continuity, which is one factor of four, rather than ruling anything out.

Does this include client-identifiable project names?

Identifying fields are stripped as part of preparing the data, before anything is shared onward.

Do you need access to the tool itself?
